Occupational Therapy Evaluation

Service Description

At Playworks, LLC, we offer comprehensive OT evaluations for your child. This may include parent report measures, relevant medical and social history, and formal/informal assessments to address the areas of concern. We will analyze the data collected, then collaborate with you to develop personalized goals and a tailored treatment plan. The length of time required for an evaluation will vary, and we may need to complete it over multiple sessions based on the child's tolerance.

Cancellation Policy

To cancel or reschedule your appointment, please contact us at least 24 hours prior to your appointment. Cancellations or no-shows within 24 hours will result in a fee of 50% of your scheduled appointment cost.
